There is a $75 non-refundable fee to submit an application which is paid by credit card when applying online or check or money order if applying via mail or fax (you may qualify for a waiver if you are applying for the Indiana Choice Scholarship Voucher). If the application fee creates a true financial hardship for your family, a limited number of waivers are available. Please call the Admissions Office 219.427.0960 to discuss the possibility of a fee waiver.

Families must submit Part I: the Application Form, which we prefer to be completed online, proof of residency & income (if applying for Indiana Choice Scholarship Voucher) & Instructional Fee. You will receive an email confirming receipt of your online application. Please note: Part II materials (teacher recommendation, school transcript, test results, etc.) do not need to be submitted with the application. Applying to Mosaic Middle School online is easy, environmentally friendly and economical. Once payment is submitted you will be directed to the online application. If the application fee creates a true financial hardship for your family, a limited number of waivers are available. A recommendation letter from a teacher is required. This letter should be written by the current Math or English teacher and submitted directly to the Enrollment Specialist by the teacher.
The essay will help us learn more about the student. It is a requirement for all applicants.
A student is accepted for admission when an acceptance letter has been received by the school and Part I items have been submitted.
2012-2013 Tuition and Fee Schedule
The school is committed to the proposition that no qualified student wishing to pursue the educational opportunities available at Mosaic be denied that chance due to financial hardship. Tuition is $4950, however we are providing a limited number of $4500 Indiana Scholarships to each student that qualifies. The scholarships are solely based upon income and are issued on a first-come, first-serve basis. Payment plans are welcome.
