Please bring the following documents with you when registering your child:
State Issued Photo ID; Shot Records; Birth Certificate; Social Security Card;Proof of Income (check stubs, unemployment document etc.); Proof of Residency (lease, mortgage statement, utility bill); Last report card and/or ISTEP scores
**School Records will be requested by Mosaic Middle School.
